Preceptor evaluations in a Keiser practicum: week 4 and week 8
Every 8-week Keiser clinical course has two formal evaluations on the Clinical Evaluation Form: a midterm in week 4 and a final in week 8, completed by your preceptor, with a column for your own self-evaluation. At week 8 you also evaluate the preceptor and the site, and in the FNP program the minimum passing rating climbs as you move through the five clinical terms.

Two evaluation points in every clinical course
Keiser's older Graduate Nursing Programs Clinical Manual schedules the preceptor and student evaluations at the middle and the end of each 8-week clinical course: week 4 for the midterm and week 8 for the final. The same form serves both times, so the midterm shows your preceptor, and you, exactly what the final will measure.
Counting straight from the start date, a practicum that opens on August 31, 2026 reaches week 4 on September 21 and week 8 on October 19. Course faculty set the exact due dates for your section, and theirs are the ones that count. These ratings sit beside the faculty's own site visit evaluation, which happens at least once a semester.
Put week 4 and week 8 on your preceptor's calendar before the term starts. The ratings come from them; the reminder should come from you.
What the Clinical Evaluation Form asks for
The Clinical Evaluation Form carries your preceptor's ratings and a separate column for your self-evaluation, which turns each evaluation into a comparison as much as a grade. When the two columns disagree, that gap is the most useful thing on the page, and it is worth talking through with your preceptor before the form goes to faculty.
Your preceptor sees the standard before the first patient. Keiser gives preceptors the Preceptor/Mentor Agreement together with the course objectives and the evaluation criteria. Keiser's public documents do not reprint the full form or its rating scale; the 2023 policy manual places graduate nursing forms on the Keiser University Academic Portal. Open the form in week 1 and read the criteria with your preceptor then, not at week 4.
FNP minimum ratings by clinical term
The older Clinical Manual sets a minimum rating on the FNP Clinical Evaluation Form that rises as you progress through the program. A rating that passes in your first practicum falls short in your last.

Two cautions apply. The manual states the minimums by clinical term, not by course number, so the course column is a reading of the catalog's sequence rather than a Keiser table. Week 4: the midterm as an early warning
The midterm lands when half the term is gone. In a 120-hour practicum spread evenly across 8 weeks, that is roughly 60 hours logged. A weak midterm rating is information you can still use: ask the preceptor which criteria fell short, agree on what the next four weeks need to show, and tell your course faculty so they hear it from you first.
The 2023 policy manual runs a parallel check on hours, with your academic advisor reviewing direct-care hours at the midpoint of clinical rotations. The same manual allows an action plan for a student found deficient at a clinical site, one that could include repeating the rotation. Week 8: the final rating and your review of the site
The final evaluation closes the term. If your preceptor's evaluation comes back unfavorable, the older manual leaves the decision on whether you met the course goals to the faculty member, not to the preceptor alone. FNP clinical courses need a B or better, and failing the clinical hours means taking the whole course again, clinical hours included. Helping your preceptor finish Keiser's evaluations
Keiser's preceptors volunteer their time, and the evaluations are paperwork on top of patient care. Put both dates on the preceptor's calendar during week 1, have the form ready when the day comes, and ask for a few minutes at the end of a clinical day rather than between patients. Clinical faculty orient new preceptors to the role, so a first-time preceptor has a Keiser contact for questions about the ratings.

WHNP clinicals such as NUR612A and NUR615A carry their own course objectives, so ask WHNP course faculty which form and which minimum ratings apply.
